Not sure if bug or just a funny thing since we don't have many ghosts yet... but this is the second time I've met this wraith with this character, on the same floor, even!

Yeah, file this under "not enough ghosts yet". Since failures to spawn ghosts due to unavailability result in whatever you used to do it being lost with no effect, I didn't want to add extra ways it could fail, so there is no checking to see if you've already seen a ghost on a given character already. Hopefully this problem will solve itself as we get more ghosts, though I acknowledge that we don't have many players who die regularly in the upper floors at the moment so in those cases it may be slower to change.
