Yeah, file this under "not enough ghosts yet". Since failures to spawn ghosts due to unavailability result in whatever you used to do it being lost with no effect, I didn't want to add extra ways it could fail, so there is no checking to see if you've already seen a ghost on a given character already. Hopefully this problem will solve itself as we get more ghosts, though I acknowledge that we don't have many players who die regularly in the upper floors at the moment so in those cases it may be slower to change.
